The Utah Vaccines for Children Program provides vaccines to participating providers for children birth through 18 years of age who are:

* Enrolled in Medicaid

*Enrolled in the Children's Health Insurance Program

*American Indian/Alaskan Native

*Not Insured

*Under-insured (insurance does not cover immunizations)
